Dual-Diagnosis Assertive Community Outreach Team Program Completion Report

This report details the activities of the Dual-Diagnosis outreach health team which seeks to enable chronically homeless/at-risk individuals with a dual diagnosis of mental illness and drug addiction to acquire and retain safe, affordable and stable housing and to receive appropriate treatment. The report provides client profiles, worker outcomes, client outcomes, and a program summary as well as recommnedations for further such projects.
