Discharge planning is a process that occurs while the individual is still incarcerated, which prepares the individual for her or his re-entry into the community. Discharge planning is a formal function of corrections administrations in several states, and occurs informally in others via correctional health providers, community-based social services providers, or other prison-based social services staff. Discharge plans usually include an estimated discharge date, programs that the individual has completed in prison, and
