The Reaching Home Community Homelessness Report (CHR) is intended to support communities in receipt of Reaching Home Designated Communities stream funding to transition to an outcomes-based approach to preventing and reducing homelessness. The report outlines the work our community has undertaken to prevent and/or reduce homelessness, details our progress towards achieving Reaching Home requirements, and lays out some of the data captured in our Homelessness Management Information System. The CHR, implemented in June 2021, gives communities a year-over-year picture of the state of homelessness and the system in place to address it in their geographic area.
