Action Mediation Program Final Report

The Société de Développement Social piloted a project titled Action Médiation: programme d’intervention psychosociale en milieu urbain.
 
The objective of this 12-month project was to pilot and test an innovative intervention model, which includes a model for a coordination and collaboration agreement between the various partners, with the aim of reducing and preventing homelessness among people who use this urban space.
 
The project builds a common project to reach homeless people in order to help and accompany them in their social reintegration process while creating a synergy between the services offered (peer helper, shuttle, mobile clinic, etc.) and the development of an optimal cohabitation between all the stakeholders of this community.
