Case Coordinator - Transitional Housing Services

The Case Coordinator supports women in addressing their immediate needs and ensures that all services/resources for which they are eligible are accessible to them. This position wo rks closely with Housing Programs and other community agencies to assist women in finding safe, affordable, permanent housing with the appropriate level of supports.

Responsibilities include:

  • Provide case management for women who a ccess the program;
  • Provide support in accessing and connecting women/families to appropriate community resources to reduce their time in the shelter system;
  • System navigation, crisis intervention, telephone support, referrals, intakes and documentation;
  • Work with women/families to resolve issues related to barriers to securing housing;
  • Completion of client documentation and case notes

Qualifications:

  • Minimum BSW or other Human Services Degree; minimum 2 years related experience
  • Bring a strong knowledge and work experience specific to women experiencing homelessness including poverty, trauma, domestic violence, addictions and mental health;
  • Knowledge of the “Housing First” philosophy and housing models and Case Management expertise
  • Knowledge and skills in crisis intervention and supportive counseling
  • Knowledge of the issues facing mothers and their children experiencing homelessness;
  • Ability to work as part of an multi - disciplinary organization;
  • Knowledge of community resources;
  • Excellent communication skills;
  • Demonstrated ability to work as part of a team and independently as well
  • Proficient in the use of computers and software including Microsoft Word
  • Current CPR and First Aid Certification
  • ASSIST Training an asset
