The London Health Sciences Centre Research Inc. piloted a project titled No Fixed Address Version 2 Expansion (NFA v.2x).
 
The objective of this 11-month project was to pilot and conduct a research project to further refine the No Fixed Address (NFA) strategy for reaching and supporting patients during the crucial transitional period when they are being discharged from the hospital and re-integrated into the community.
 
This project represents a potential best practice model of discharge that can be adopted throughout Canada, with direct implications for care providers, service delivery, and the wellbeing of the individuals at risk of homelessness.
