The Centre intégré universitaire de santé et de services sociaux du Centre-Sud-de-l ‘Île-de-Montréal (CCSMTL) piloted a project titled Programme Réaffiliation en Itinérance et Santé Mentale (PRISM) with Accueil Bonneau.
The goal of this seventeen-month project was to implement a program to help chronically homeless people with mental health problems to move off the streets and find residential stability.
The project offered transitional housing and intensive psychosocial support to the targeted clientele, with the possibility of on-site medical and psychiatric assessment, treatment and follow-up. Through this partnership, the CCSMTL and the Accueil Bonneau wanted to respond to a wide range of issues related to a marginalized and socially excluded population that is resistant to emergency accommodation and care services, for both physical and mental health, offered by the health network services.
This project was funded in part by the Government of Canada's Homelessness Partnering Strategy’s Innovative Solutions to Homelessness program. The opinions and interpretations provided in the publication are those of the author and do not necessarily reflect those of the Government of Canada.
